Add smt keymap for Clueboard (HHKB-style layout) (#1286)
* Add keymap for smt Clueboard (HHKB layout)
* Add readme for smt Clueboard (HHKB) keymap
* Flesh out the keymap a bit more to support Colemak & Dvorak
* Update README with layout image
